A draft 'Independent Living Mini Survey' aimed at people with disabilities, family members, caregivers, advocates, and disability service providers. It uses plain-language, accessible questions asking who the respondent is filling it out for, which needed services they are NOT getting (rights, emergencies, benefits, social, work, school, housing, transportation), how hard it is to get help, and which Hawaii county they live in, plus optional contact information and an open comment field.
